Question:

I have a large bulb at the tip of my nose. I am interested in rhinoplasty and when I look at before and after pictures online, I see the types of changes that I want done to my own nose. Can my nose job be done entirely from inside the nose

Answer:

A Rhinoplasty can be performed entirely from the inside of the nose and this is referred to as an ‘endonasal’ or closed Rhinoplasty in most textbooks. There are some advantages to having an endonasal Rhinoplasty, primarily the fact that the recovery is lessened and it is less invasive. In our practice approximately 80% of primary rhinoplasties are performed using this approach for the potential benefits mentioned above. In a small percentage of patients, we do feel the open approach is advantageous depending on what needs to be accomplished. The best way to determine this is to seek a consultation with an experienced Rhinoplasty surgeon who uses the endonasal technique.

Question:

I had a lipoma removed from my arm and it left a rather large, very noticeable scar. I am 63 years old, a proud old Southerner and wear sleeveless shirts most of the time. The scar always catches my attention and I am beginning to hate it. What procedure do you use for scar removal?

Answer:

The procedure used to remove a scar is what is referred to as a ‘scar revision procedure’. This typically consists of removing the old scar and replacing it with a surgically designed scar camouflage type procedure. Using modern techniques and scar revision procedures, this can often be made to look considerably less noticeable as compared to the previous scar. It is important that the patient understand that it will always look worse before it looks better after having a scar revision procedure, but it is possible that improvement could be accomplished. Furthermore, many patients who undergo scar revision procedures can have some additional degree of improvement with scar camouflage using laser abrasion or dermabrasion.
