Question:

My 11-year-old daughter is quite self-conscious of her ears because they stick out.  Is she too young for otoplasty? How long will she have to stay inactive afterwards? She loves sports.

Answer:

The age of 11 is actually not too young for Otoplasty. Most patients seek who have otoplasty performed are either five  years of age or in the early teenage years.  Five is a nice age because the ears are almost of adult size and the child has not had much opportunity to be ridiculed in school as some children are who have prominent ears.  Incisions are placed behind the ear so that it is virtually undetectable.  We typically like patients to stay out of contact sports for approximately six weeks, however, they can resume vigorous activities in about two weeks.  The procedure takes approximately one hour for each ear and is performed as an outpatient.

Question:

I have loose skin around my jaw line, as though my cheeks are dropping into my jaw.  I would like to have the weekend lift. Do I have to have the incisions in front of my ears? I really don’t want scars there.

Answer:

The weekend lift is really the best procedure to address some early jowling or an aged jawline.  The incisions are placed in a very inconspicuous area behind the ears.  For women who like to wear their hair up, it is important that these incisions are not seen in public.  Also for the athletic patient, i.e., tennis player, golfer, it is important to be able to wear their up and not have these incisions noticeable.

Question:

I had a dog bite when I was a child. The scars are still visible.  What procedure do you use for scar revision? Can this be done in the office?

Answer:

It is difficult to answer the question about scar revision surgery without first seeing the patient. There are certain types of procedures that can be used to soften the scar which we refer to as laser abrasion or dermabrasion.  For more significant scars, it is important to actually surgically remove the scar first and repair it in a controlled fashion.  Typically the approach that we use on the face is what we refer to as a geometric broken line repair or a z-plasty.  This is a procedure that we have published on and it can be found in the literature.  Essentially the procedure is performed to take control of the scar and hide it in the area where the eye does not expect to see a line.

Question:

I am a fairly young woman, 29 years old, yet have hooded eyes.  These eyes are inherited from my dad’s side and I think they make me looker older and tired.  Can blepharoplasty remove this excess skin? Where is the incision made?

Answer:

A blepharoplasty can remove the excess skin of the upper eyelids. The incision is placed in the first crease of the upper eyelid as you move away from the eyelashes.  This way when the area is healed, the incision is virtually undetectable.  It is true that excess skin which is inherited does make someone look tired even at a younger age in their 20s.

Question:

I have a flattened bridge on my nose because it got broken three years ago. I would like rhinoplasty to get the flattened area reshaped and the tip brought up.  Can this be done with a closed procedure? Is general or local anesthesia used?

Answer:

A flattened bridge is often the result of trauma and it is what we often refer to as a saddle-nose deformity.  It gets t his name because it can resemble a saddle if you were looking at a saddle from the side where it settles in the mid-aspect of the nose.  A closed Rhinoplasty or what we often refer to as an endonasal Rhinoplasty procedure is one approach that is used in Rhinoplasty.  In our practice it happens to be an approach that we use in almost 80% of primary Rhinoplasty.  This is a procedure where all of the incisions are placed on the inside of the nose and has a significantly shorter recovery period with less trauma to the nose and to the patient.  The anesthesia that is used is a twilight anesthesia similar to someone having their wisdom teeth removed or a colonoscopy.  The procedure is performed in our Surgery Center as an outpatient.

Question:

I need neck work.  The skin hangs down around my neck in folds, and there is fat. Where is the incision and is the neck muscle actually tightened?  I also want to know if you can take out some fat at the same time.

Answer:

When the procedure you are talking about is what we refer to as a weekend lift and a necklift with liposuction.  The liposuction of the neck below the jawline occurs through a small incision that is under the chin area.  Routinely, the muscle is tightened in that area so to begin creating a sling that could be tightened from the incision around the ear.  The necklift is almost always performed with a weekend lift which addresses the neck and the jawline at the same time. This allows contouring of the jawline as well as the neck and a deeper muscle suspension is performed so as to perform a virtually undetectable scar around the ear by using a deeper repair of the muscle.

Question:

Which hormones can you test using saliva testing?  Is it possible that a woman can be low in testosterone?

Answer:

Yes, I always test my hormones through saliva testing because this will test the Bioavailable amount of hormones in the body, not the total amount, which is sometimes inaccessible for the body to use.  Saliva testing is done in a timed fashion and we usually, depending on the patient, will do it in one day or over the course of a month.   The hormones I typically test using saliva testing includes Estrogen-Estradiol, Estrone, Estriol, progesterone, testosterone, BAGA, Cortisol.   These are typically measured in saliva tests.  For thyroid hormone, I do measure this in a blood test. To answer the second part of the question, yes, it is very common for women to be low in testosterone.  About 80% of women are low in testosterone as they approach perimenopause and beyond.  There is a small percentage who are high-testosterone and we have to also analyze these patients as well.  With high testosterone, you can have abnormalities in glucose and insulin, and possibly have polycystic ovarian syndrome, or be at high risk for heart disease.  Patients with low testosterone tend to have a low libido, lack of muscle tone, and drive, and benefit significantly from Bio-identical testosterone replacement.

Question:

I think menopause should be a natural part of aging, not something to fear.  Do you have a variety of natural products to use to alleviate the symptoms of menopause?

Answer:

I agree, menopause is a natural part of the aging process, and not something to fear.  It is a wonderful transition for women, and most women come out the other side of menopause confident, healthy and feeling well.  However, during the time of perimenopause can be so severe that they will interrupt the quality of your life.  In this case, I think it is important to use natural treatment to help feel the best that you can.  I favor Bio-identical or natural hormone replacement because this is very effective, and has multiple health benefits for women who are lacking hormones.  If that is not something that you would want to do, I would recommend using some vitamins and herbs that can help during the time of menopause such as black cohosh and chaste berry.  These offer help with the mild symptoms of menopause but will not provide the health benefits that Bio-identical hormone replacement does.   Remember, estrogen is your hormone of memory, it is your hormone for vision and heart health.  It helps maintain your bones and helps prevent osteoporosis.  Progesterone helps anxiety, moodiness, depression, dizziness, and testosterone helps muscle tone and sex drive.  These are all important functions that women want to maintain throughout their life.  That is why Bio-identical hormone replacement is a very healthy treatment for perimenopause and beyond.

I would like to consult with you about skin products and diet for an anti-aging regimen.  For the diet, are there special foods required, or is it more about choosing better foods at the grocery store?

Answer:

The optimal diet for anti-aging is one that incorporates fresh local organic foods into the diet.   In particular, organic fruits and vegetables and meats are extremely important to eat, as well as all of your foods being pesticide-free.  Pesticides and nonorganic foods are toxins to the body and can make the body chronically ill over time.   Organic foods are not always available and in that case, wash your foods very well and minimize the amount of nonorganic foods that you eat.  You can choose better foods at the grocery store and most mainstream grocery stores now have many organic and healthy options.   Some specialty foods are available at health food stores as well.  Besides eating clean, simple, natural fresh foods, there are no other special or magic foods to incorporate into an anti-aging regimen.  You want colorful vegetables and fruits; you want organic lean protein, organic nuts, organic olive oil and whole grains to have a healthy balanced diet.
