My cousin has been on testosterone therapy to get more testosterone. Is this safe?

Answer:

Testosterone therapy prescribed by a knowledgeable doctor is certainly safe but it is only used to replace a deficiency in testosterone. It should never be used to enhance sports performance or build muscle or be abused as is very common today by athletes, etc. The only reason to take testosterone is if you have a testosterone deficiency and then you want to have your levels be within the optimal range and not exceed that. Even though bioidenticals are clearly safe and effective, if you are using them incorrectly, they can also have consequences to them.

Question:

I started on BHRT because my friend swore by them. I have to admit, she looks better, has more energy and seems so much happier. I have not had any real change yet. Is this normal? Is it possible they will not work for me like they have for her?

Answer:

The thing with bioidentical hormone replacement is that it takes time. It is not a magic bullet and it is not an overnight change. It is not a pharmaceutical that instantly creates a change in the body. You really have to wait until your blood levels and tissue levels increase and that can take up to three months' time after you start them. The key is patience. You just have to stay patient. When it has been three months, you can retest your levels and then have your doses adjusted as necessary. There is that sweet spot where you find the right dose for you and you will reap all the benefits that bioidentical hormones can provide.

Question:

My friend's doctor prescribed her bioidenticals but my doctor does not do this. Why is there such a difference here? Who am I supposed to get them prescribed by?

Answer:

This is a huge challenge facing physicians and patients today. The physicians who are not familiar with bioidentical hormones, are not interested in bioidentical hormones or have not extended themselves to receive training in bioidentical hormones, will not likely prescribe you them because of their lack of awareness of the benefits, how to dose them, etc. Because they are not pharmaceuticals, there is no advertising by the pharmaceutical industry, there is no education by the pharmaceutical industry of doctors regarding these so it is something that the doctors who are interested in have to go out and pursue their own education in bioidentical hormone replacement. This does not apply to many physicians. So, the best way to find a doctor who is familiar with bioidentical hormone replacement is to either contact the American Academy of Anti-Aging Medicine or looking on online resources to find a local anti-aging and regenerative medicine doctor near you. This could also be a gynecologist or functional medicine specialist. However, it does take a little research to get to the right physician.

Question:

When you do BHRT via pellets, what happens if your hormone levels are too high? Is the pellet removed?  Is this a surgical procedure?

Answer:

The reason I do not use pellet is because yes, it is a surgical procedure and you do get less consistent levels of hormones in the body. When first inserted, the hormone levels may be too high and the only way to stop that is to remove the pellet which can create a scar and scar tissue upon repeated insertions and removals. Some people like them very much; it is just not a part of my practice.

Question:

What is the difference between bioidentical hormones made up in compounding pharmacies and those you get in standard FDA approved doses from drug companies? Everyone seems to taut the compounding, but I would much rather take something that is FDA approved. 

Answer:

This is a complicated issue. The benefits of using a compounding pharmacy are first you can use Estriol which is a weaker type of estrogen that is naturally found in women that have some very protective effects for the breast and can help replace deficient levels of this hormone in the body and this is not available by standard FDA approved pharmaceuticals. The compounding pharmacies make up the prescription uniquely for you so the dose can be adjusted very precisely to match your unique needs which are why I favor compounding pharmacies, both for the Estriol availability and for the individualized dosing. FDA approval process really only applies to pharmaceutical companies. It cannot truly apply to a compounding pharmacist who is making up a prescription one at a time. It is like comparing apples and oranges, it is a different regulatory body to some degree. However, I do on occasion use the pharmaceutical bioidentical hormones such as the Climara patch, Vivelle Dot, Prometrium and for some patients where there are financial constraints or they have a preference for this type of pharmaceutical, it is an option that is available to you.

Question: Can bioidentical hormones be used instead of birth control in girls to regulate their period and help with cramps?
Answer: Bioidentical hormones are not going to provide birth control however in young women that have PMS or heavy cramping and are deficient in progesterone for example, hormones can be helpful to be used cyclically, i.e., in the second half of their menstrual cycle to help supplement waning progesterone levels. However, they certainly do not find a means of birth control in and of themselves but they are a nice alternative for patients to avoid some of the side effects of the birth control pills that are deficient in progesterone.

Question:

I am a man and I have been considering bioidentical hormone therapy. I have thinning hair and I was wondering if this could either stop the hair loss or reverse it in any way?

Answer:

Unfortunately, male pattern baldness is a tricky condition to reverse. Certainly by balancing out your hormones, this can be beneficial for hair loss in general, however, I rarely see any true regrowth in men from bioidentical hormone replacement therapy.

Question:

Can facial hair growth occur from taking bioidentical hormones?

Answer:

Yes, it can if you are taking doses that are too high.  Primarily in women that have high testosterone or high DHEA levels, this can commonly result in the production of facial hair growth. However, if your hormones are well balanced, this usually helps control any facial hair growth that many women see around the time of menopause.

Question:

I feel like I have been retaining water since I have been on BHRT. My face actually looks puffy. Is this caused by the hormones?

Answer:

It is possible. Certainly a hormonal imbalance can cause water retention. It could be that your estrogen or progesterone levels are too high. They may be out of balance with each other. Some men that are on very high doses of testosterone can retain fluid as well. If this happens, it is certainly important to come in and see your doctor immediately, have your levels checked and make any dosage adjustments as needed.

At what age in men and women are the hormones at an optimal level? With BHRT are you trying to return the body to those levels?

Answer:

In general, about the age of 25 hormones are at their optimal level in men and woman, and yes with BHRT we are trying to return to the body to those optimal levels at their peak production, approximately age 25.
