Question:

Can you explain what pellet therapy is? Is it a good thing?

Answer:

Pellet therapy is where a small pellet is inserted under the skin that will release hormones over a specified period of time. Regarding whether it is a good thing or not, I think it is more patient preference. I personally do not use it in my practice because it does require a small surgical procedure to insert the pellet. Should it be the wrong dose it is difficult to remove and it just is more laborious than using a transdermal cream. Additionally, transdermal creams used every 12-24 hours are going to give you a more even hormone release in the body than a pellet would.

Question:

Is BHRT the only alternative to traditional hormone replacement therapy like Premarin or Prempro?

Answer:

I feel that BHRT is the only wise alternative to traditional synthetic hormone replacement because of its safety, because of its effectiveness and because of the health benefits it provides. Some women elect to use herbal therapy to help control mild symptoms of menopause which I think is very reasonable to do however you will not get the benefits of the actual hormones as you would if you were on BHRT.

Question:

I had a complete hysterectomy at age 47 and have had no luck for improving my symptoms. Could BHRT help me?

Answer:

Absolutely. In women that have had a complete hysterectomy which is removal of the uterus and ovaries are usually quite hormonally depleted and start to not only age more rapidly but gain weight and have more severe menopausal symptoms. Bioidentical hormone replacement can mitigate and decrease these symptoms and also provide the health benefits of the hormones that you are lacking.

Question:

Is there a certain time of day that is best for applying bioidentical hormone replacement therapy cream for menopause?

Answer:

Any time of day is fine to use bioidentical hormone replacement. Some women find estrogen and testosterone to be stimulating and therefore elect to use it in the morning. Others find progesterone to be relaxing and elect to use that in the evening. The time of day that you can commit to being most consistent with is probably the best time of day for you.

Question:

What kind of results can I expect with BHRT and how soon after I start treatment?

Answer:

The results you can expect include reduction or elimination of menopausal symptoms such as hot flashes and night sweats as well as the health benefits of being hormonally balanced. These include bone health, memory health, improved growth and repair of the body, heart health and feeling an improved quality of life. These benefits for symptom control may take up to three months to be fully realized. You may feel better much sooner than that however it does take about three months to reach a good foundational level of hormones in the body.

How can BHRT help with perimenopausal and menopausal symptoms?

Answer:

BHRT supplies the body with the hormones that it is lacking. Typically during perimenopause and menopause there is a lack of estrogen and progesterone and sometimes testosterone and DHEA as well. By supplementing these hormones to premenopausal levels, many symptoms of menopause are controlled such as night sweats, hot flashes, memory loss, weight gain, osteopenia, osteoporosis, etc.

Are there any risks associated with BHRT?

Answer:

If BHRT is done systematically, carefully and judiciously, the risks can be minimized if not removed completely. Patients with breast cancer themselves are not candidates for BHRT however other women are candidates and in fact will derive multiple health benefits by being hormonally balanced. Using BHRT requires annual screening, especially a mammogram as well as a pap smear, and monthly self breast exams.

Question:

How is BHRT supposed to be better than synthetic hormones?

Answer:

BHRT is superior to synthetic hormones for many reasons primarily because the chemical structure of bioidentical hormones is the same as the chemical structure of hormones in the human body which makes them bioidentical. Some of the synthetic hormones used, estrogen is derived from equine sources which are not bioidentical. It is believed that having hormones that the body can recognize as its own allows them to be metabolized and utilized appropriately without side effects as seen from some of the synthetic hormones.

Question:

Help! I am moving overseas in a few months and I have just started taking my BHRT. Will it be possible for me to continue my regimen abroad or is this something that needs to be closely monitored by my doctor?

Answer:

I have had patients go abroad for a year or more and as long as you are able to speak by phone or to touch bases and to have your routine health screening, i.e., mammogram and pap smears done, there is no reason you should stop your bioidentical hormone replacement therapy. This can be shipped anywhere in the world from your local pharmacy and you may continue it safely. You could also find a doctor in your new location and have them monitor you as well

Question:

I am a 51-year-old woman going through menopause and would like to start some sort of hormone therapy. What kind of tests are done to determine if I would be a good recipient of BHRT?

Answer:

I would do a saliva test. If you are perimenopausal, this can be done on multiple days throughout the course of a month or you can do a one-day test. There are other options as well however in my practice I tend to favor saliva testing for its convenience and accuracy. This would be the best test to do to give an idea of your hormonal losses and what doses you might need should you start bioidentical hormone replacement.
