Question: I lost quite a bit of weight last year and now I am into body lifting. I am a male, 47 years old and very active. I have been able to improve my muscles with conditioning and most of my body looks great but I am interested in competition and all of my body must look great. I have skin that sags off my upper arms. Would a brachioplasty result in a scar that would show in competition? Answer:
A brachioplasty is a procedure that involves removal of skin and fat off the arms. Very often patients that have lost a lot of weight have a very wide area of skin hanging off the arm, very often referred to as ‘bat wings”. Removal of this excess skin and fat can provide a very nice contour and shape to the arms; however, it does involve a scar that would go from the armpit down to the elbow on the inside. When someone has their arms at rest down by their sides, this scar should not be visible from the front or the back. However, if you lift up your arms or if you are competing, then these scars would be visible. After about six months to one year, these scars have usually faded to the point where they are not very noticeable. Many people who have excess skin inside of the arms, only have this excess fat in the upper arms near the armpits. In those cases it may be possible to do a procedure that involves only liposuction and lifting the arms with an incision only in the armpit. In those cases that scar can be easily concealed since it does not go down the entire length of the arm. But in most cases, a brachioplasty does involve an incision that goes from the armpit to the elbows but with time, massage and pressure treatment, those scars usually fade to the point where they are not noticeable and do not become a problem. Question: I have a severe muffin-top where my stomach hangs over my pants. It was bad before I became pregnant, but really got worse after I had a baby. I am looking into a tummy-tuck. Does abdominoplasty involve the muscles of the abdomen or just the skin and fat? Answer:
The way we perform a tummy-tuck involves three things. First, the extra skin and fat of the tummy below the belly button is removed. Second, the muscles underneath are tightened up because these muscles get separated with pregnancies or massive weight gain or weight loss. Finally, liposuction is used as a tool to sculpt the Abs, the waist and the back, in order to get the best contour. All of these three things are necessary to get the best results with a tummy-tuck. Question: My breasts are too large and not proportional to the rest of my body. I get severe backaches almost daily and I am only 26 years old. I hate having my breasts stared at whenever I go out and it is turning me into a hermit. Do many women my age get breast reduction surgery or should I wait until after I have children? Answer:
Large breasts are a significant problem for many women your age. Even before having children breasts can become so large that they become a social problem and also interfere with physical activities and sporting activities. It is very common to do breast reduction surgery in patients before they have children; however, it is very important to understand that your breasts will still behave the same way with pregnancies even after breast reduction – meaning that if there is a significant amount of weight gain with a breast reduction or if you choose to breastfeed, the breasts will enlarge again. However, they will never become quite as large as before the breast reduction. Many women who have had breast reduction and then go on to have pregnancies, if they keep the weight gain to a normal 25-30 pounds, they do not see significant changes in their breasts. Breast reduction surgery is one of the most satisfying procedures for patients since the weight is gone immediately and the back pain is alleviated almost immediately, even though there may be some discomfort from the actual surgery in the first couple of days, the weight is lifted and there is immediate relief of the strain the breasts were causing on the shoulders. Question: I do not like the way my breasts look. My breasts sag and look like what I would imagine an ‘old lady’s’ breasts look like. I am interested in a breast-lift. Can I get breast augmentation at the same time? Answer:
Many women have problems with breasts that are sagging either after weight loss or pregnancies and breastfeeding. Sometimes women have enough breast tissue, the only problem is that the breast tissue is hanging too low and it is possible in these patients to do a breast-lift where all the tissue is used and sculpted in order to make a nice shape and provide good cleavage. The breast is also re-suspended where it belongs and we use a technique where an internal hammock is created with your own tissues on the inside to provide support for the breasts. So in many cases it is possible to have a very nice result using only your existing breast tissue without an implant. But, however, in some cases there just is not enough breast tissue to create a full look. Therefore, in those cases we have to perform a breast-lift and at the same time place an implant under the muscle to provide additional volume. So these procedures are very often performed at the same time and provide excellent results. Posted by

Question: I would like both an abdominoplasty and breast augmentation. Is it safe to have both procedures done at the same time? Answer:
A tummy-tuck breast aug is a combination of procedures that is very commonly done. Very often women have concerns about their tummies and their breasts and when these procedures are performed at the same time, they do compliment each other and the overall result is even better. The way we are able to do th is safely is, of course, to make sure the patients are in good physical health before the surgery and after the surgery pain is a concern. Therefore, we use techniques of anesthesia that allow us to do this on an outpatient basis and one of these things that have offered significant comfort to patients is a pain-pump which is a device that automatically delivers a local anesthetic therefore keeping the operative areas numb for 3-5 days after surgery. With these techniques we are able to do combined procedures of the tummy and breasts safely and comfortably. Posted by

Question: I am a 48 year-old professional bodybuilder so I have to look good in a speedo. My buttocks despite years of working on them are almost pure muscle but are too flat. Can a buttock-lift improve or enlarge the shape of the buttocks or just lift up what is there? Answer:
A professional bodybuilder who has good muscle tone but very little fat could be a good candidate for a lift in two different techniques. First, even though there might be very little fat around the body, we could still harvest some fat and perform what is called a Brazilian Butt-lift. This is a procedure where fat is taken and prepared from other areas of the body and transferred to the buttocks to enhance the shape of the buttocks and the contour. Again, since this is done by transferring fat, there are no big incisions and no big scars. However, if someone does not have a lot of fat to give and there is not sufficient fat to perform a Brazilian Butt-lift, then buttock implants can be used. These are essentially silicone implants that are more firm but still very soft. These are placed under the muscle of the buttocks through a small incision through the buttock cheeks. These implants provide a very natural look but a very full contour and are good options for patients who do not have a lot of fat. If a person is a bodybuilder and has good muscle tone, then actually lifting the buttocks with an incision above the buttocks may not be necessary. Posted by

Question: I was obese four years ago but I have lost a lot of weight and I am stable now. The problem now is, of course, that I have a lot of loose skin around my waist, thighs and buttocks. I have read that there is a procedure that would correct all three areas, a body lift. How long does this procedure take? Is it done under general anesthesia? Will there be any scarring afterwards? Answer:
People who have lost a lot of weight whether on their own or after gastric bypass can be ideal candidates for a body lift. A body lift is essentially a tummy-tuck, thigh-lift and buttock-lift all at the same time. The incision for this would go all the way around, starting at the pubic area extending around the hips all the way to the buttocks. The scar is placed along special lines so that it will fall right under your normal underwear or under garments and will not be seen. The procedure may take 4-6 hours and it is done under general anesthesia. The scar again does go all the way around but because it is placed very low it can be hidden under under-garments or under normal clothing. It is a perfect procedure for someone who has lost a massive amount of weight and has excess skin and laxity along the tummy, thighs and buttocks since it would take care of all three areas all at once. Question: I think my breasts are in good shape but it looks like my nipples point south. Can my nipples be relocated to another site while I am having my breast augmentation surgery? Answer:
Many women who need breast augmentation surgery also need a breast-lift. Sometimes the nipples are drooping below the fold under the breasts and, therefore, they will still continue to look like they are drooping after the breast augmentation if a lift is not performed. The lift would involve reshaping the actual breasts and repositioning the nipples higher so that they are situated directly over the implants and do not appear to droop. So, these procedures can be commonly performed together and the implant can be placed through the same incision that is used to do the lift. Therefore, there would be no other incisions on the breasts. But a breast-lift with a breast augmentation is a very good procedure to do at the same time. Posted by

Question: I am a 32 year old female and I have terribly small breasts. I am big boned, but of normal weight. I would like breast augmentation. Do you make recommendations on cup size? I do not want to look like I have had breast augmentation. Answer:
When considering a breast augmentation, cup size is a very personal thing, but whether you would like to be a B or a full D-cup I believe the most important thing is to have the breasts look and feel natural. That’s why I place the impalnts under the muscle, giving the breasts a softer, more natural look and feel. Even if you chose to have larger implants it’s still very important that they look natural.
