Bariatric Surgery  

        Due to the popularity and success of bariatric procedures such as the open or laparoscopic gastric bypass and the lap band, we are witnessing today dramatic levels of weight loss.  It is not uncommon to see patients losing 100 to 200lbs at one year after surgery.  While it cannot be overstated how beneficial this weight loss is when it comes to medical and physical well being, the consequences are that patients are now faced with laxity and excess skin the abdomen, breasts, arms and thighs.  Some patients even feel that the excess skin problem is more of a concern than being overweight.  Dr. Polynice believes that it is important for a patient considering gastric bypass to consult with a plastic surgeon before proceeding with the weight loss surgery.  The information obtained at that consultation could be invaluable.  The patient can be more informed and know what to expect once their weight loss goals have been achieved.  Dr. Polynice believes that ultimate success in the bariatric patient is not only measured in pounds lost but also in the final esthetic result that can be obtained.  The best result will be obtained when the bariatric patient is well informed from the beginning as to what can be achieved and when he or she has the support not only from their medical team and the surgical team performing the gastric bypass but also from their plastic surgeon.

The procedures commonly performed following massive weight loss are:

  • Extended Abdominoplasty (this would include the Abdominoplasty and a lift of the anterior and lateral thighs)
  • Lower Body Lift (this procedure includes the extended Abdominoplasty, anterior and lateral thigh lift and buttock lift)
  • Arm Lift (Brachioplasty)
  • Thigh Lift
  • Breast Lift (Mastopexy, with or without implants)

These procedures are very often done in combined fashion, for example an extended Abdominoplasty can be done with an arm lift or an arm lift and breast lift can be done together.  Most gastric bypass patients however will need one or more of these procedures once their weight has stabilized, they should then be staged at about 12 week intervals.

Since I have been experiencing perimenopause, I have had occasional bouts of rapid heartbeat.  It is actually very scary.  Is this hormonal and is it anything to be concerned about? Can it be treated with hormone therapy?

Heart racing and being able to notice your heart beating can be associated with menopause. I would check your hormone levels and replace any deficient hormones and see if the symptoms resolve. Additionally, it would be wise to see your primary care physician to have an EKG performed to make sure there are no additional issues to be dealt with but typically mild symptoms are related to menopause.
