This is a 35 year old female who presents to us after previous breast augmentation surgery. The patient has saline implants in a subpectoral pocket by another surgeon. She presented with two issues. On her right breast she had significant bottoming out of the implant with the implant stretching the skin of the lower pole of the breast creating more fullness in the lower pole and making the nipple appear to be high up on the implant. On the left side she also had some bottoming out but the implant there was ruptured. The patient underwent a procedure with an incision in the inframammary fold where both implants were removed. The one on the right was intact; the one on the left was ruptured. She had a repair and reconstruction of the inframammary fold and new saline implants were replaced in the subpectoral pocket. Results at three months show very nice contour and nice position of the implant. The inframammary fold has been reestablished and the implant now sits in the proper position.

This 40 year old patient, who is 5'2" and weighs 113 lbs., presents with problems with implants placed 2 years prior. She had 400cc saline implants placed under the muscle. She developed bottoming out of the implants with the deformity known as "double bubble". She also had some capsular contractive deformities. This was corrected with a complete revision procedure where the old implants were removed, capsulectomies were done, new implants were replaced. These were textured silicone implants, 378cc's on the right and 401cc's on the left. We also did a mastopexy and used strattice which is a matrix made from pig skin. This was used to provide additional coverage and support of the implants. Results at 6 months show a significant improvement in the shape, contour and position of the implants.

This is a 24 year old female, who is 5'2" and weighs 110 lbs., presents with very asymmetrical breasts and a tubular breast deformity. Post-op photos at 3 months showing results after smooth silicone implants were placed under the muscle. The tubular deformity was corrected by opening the base of the breast. Different sized implants were used to correct the asymmetry. An inferior areola incision was used. It usually takes 6 - 12 months for these scars to fade. She went from a 32AA to a 32C cup size.

The patient developed a capsular contracture on both sides but she was also very unhappy with the size and position of the breast implants. They were too large and too high. The patient is very petite at 5”3” and 108lbs. she had moderate profile saline implants 300cc these fit her body much better giving her a more natural and soft look.

This 52 year old female who is 5’6-1/2” tall and 149 pounds. The preoperative photographs show the patient in 2001 where she had previous breast lift with 450cc silicone implants placed under the muscle. The patient was unhappy with these results. Notice how the right breast is still larger than the left and both breasts are still dropping. On the oblique view notice as there is more fullness on the upper pole of the breast and still significant dropping of the breasts or ptosis. Notice the position of the nipple which is much lower than the fold under the breast. The postoperative photo shows the patient only two months following a revision breast lift with augmentation. The existing implants were removed and these were placed with silicone, smooth, round 600cc implants.

This is a 32 year old female who had a breast augmentation done elsewhere. She presented with a breast complication which included asymmetry and bottoming out on the right side. We did a revision breast surgery where we created internal support for the implants using a mesh made from pigskin. This improved not only the position but the shape of the breast and provided a long lasting result as well. This patient returned 5 years after her surgery after giving birth and breast feeding, and her breasts have maintained their shape and contour.

A 49-year-old female, 5'4" tall, 138 pounds. She has had three pregnancies, three normal deliveries. The patient came to see us because she had drooping of her implants. These were saline implants placed 20 years ago and over the years they not only drooped but the left one had completely deflated. Notice on the preop image how the left breast is flat and deflated. The patient therefore underwent procedure for removal of both saline implants, the left one ruptured, the right one was still intact. She had replacement with new silicone implants in a new pocket under the muscle and the reconstruction was reinforced with acellular dermal matrix, in this case Strattice which is a dermal matrix made from pigskin used to support and reinforce the repair. The patient had a new 475 cc smooth, round, moderate profile silicone implants placed. Postoperative photos show result at three months with the breasts nicely repositioned with a very nice contour and correction of the bottoming out.

This patient had saline implants placed under the breast (on top of the muscle) 14 years before. These were 400 cc textured saline implants. The left side ruptured and she also had a severe capsular contracture on the right causing the implant to be displaced superiorly. The patient had a revision breast operation to remove the ruptured implant and removal of the the thick capsule surrounding the implants. Both implants were replaced with new 435 cc silicone implants placed in a new pocket under the muscle.The breasts are now soft and symmetrical. Results shown at 3 months.

This is a 51 year old woman who had a bilateral breast augmentation with me in 2014. At the time she had textured silicone implants placed under the muscle. Now that there is a correlation between textured implants and the possibility of Breast Implant Associated Acute Large Cell Lymphoma (BIA-ALCL) the decision was made to remove these implants. She wanted to restore some of the lost volume following removal of the implants but she didn't want to replace with new implants. She was a great candidate for implant removal with fat transfer to add volume. The implants were removed through her old incision in the fold under the breast and about 200cc of fat was added to each breast to restore volume. The fat was harvested from the back, the waist and the abdomen. Fat transfer is an excellent and safe technique to restore shape and volume in cases of implant exchanges or breast reconstruction. The photos taken at one year show a very nice, soft, long lasting result and she is very pleased.
