SADDLE NOSE DEFORMITY (BOXER NOSE)
Typically, the saddle nose deformity is caused by a traumatic injury to the nose and nasal septum resulting in the loss of support in the mid or dorsal region with settling of the cartilage framework and soft tissue above the tip of the nose. 
This settling looks similar to the seat of a saddle and for that reason is described as a saddle nose deformity or boxer nose. Repairing this type of deformity often requires rebuilding support in the middle aspect of the nose by using a cartilage graft harvested from the patient’s nasal septum or ear, or in severe cases from the patient’s rib. Other options, such as Gortex implants may be used depending on individual anatomy.
Nose Tip Reshaping Rhinoplasty
NASAL TIP REFINED
Many patients are interested in Rhinoplasty to improve the shape of the nasal tip. Terms used to describe this condition include a morpheous or bulbous tip and it is characterized by fullness resulting from a thick nasal skin or from large overdeveloped lower lateral cartilage.

The amount of refinement may be limited in patients that present with thick nasal skin however, when the bulbosity is the result of overdeveloped lower lateral cartilage, sculpting or refining can result in a more aesthetically pleasing nasal tip that is in alignment with the upper part of the nose.
This can often be accomplished using an endonasal technique however, in some patients it may require an external approach depending on their anatomy. Dr Williams is a board certified facial plastic surgeon and preforms surgical Rhinoplasty procedures as outpatient surgeries at the New England Laser and Cosmetic Surgery Center.

Some patients have a nose that they are dissatisfied with primarily due to a large nasal hump or bump. This is often seen in a profile examination or in photographs. The nasal hump often results from a combination of an overgrowth of the nasal bones and nasal cartilage in the lower aspect of the nose.

The nasal hump can often be corrected without making any external incisions; rather it can be accomplished using an endonasal technique where the incisions are made internally.
Large Nose
LARGE OR OVERPROJECTED NOSE
A large nose often protrudes excessively from the face resulting in disharmony. When the nasal tip stands too far from the face, it is described as over projection. The most common complaint by patients is that they are dissatisfied with their profile.

Furthermore, many patients who have an over projected nasal tip have a prominent nasal hump that also contributes to the deformity. This is corrected by under going Rhinoplasty. Dr. Williams moves the nasal tip back toward the face with a light amount of rotation and then aligning the profile with the nasal tip resulting in a straight and natural profile.

LipoDissolveTM works through a series of two to six treatments, each of which requires multiple passes of a needle. In fact, each treatment of large areas, such as the stomach, can require as many as 120 injections. The injections typically contain a mixture of phosphatidylcholine and sodium deoxycholate, the later of which is a bile salt and aids in waste removal.It is important for the consumer to also understand that although this is a non-surgical technique, each of the two to six treatments that are required can cost from $300 to $1,500, therefore, requiring a substantial financial investment for the patient. Since this is a new treatment modality, many concerns have arisen.
The officials of the American Society of Plastic Surgeons, the American Society of Dermatologic Surgeons, and the American Society for Aesthetic Plastic Surgery have brought forth two main issues. First, and foremost, is for the safety of the patients. LipoDissolveTM is prepared in compounding pharmacies, which are small pharmacies who typically prepare small quantities of drugs for specialized treatments. Quality control and sterility have been a key issue with these compounding pharmacies in the past. This concern has been brought to the forefront of discussions since numerous reports of bacterial infections, granulomas, and localized tissue necrosis have been reported following the use of LipoDissolveTM. The second issue is that, to date, no scientific data proving the effectiveness of this treatment modality has been presented. In an attempt to determine both the safety and efficacy of this treatment modality, the American Society for Aesthetic Plastic Surgery is presently sponsoring a small placebo-controlled study under the supervision of the FDA.
